Case Study:
Please find the following pictures. I don’t have much detail. Still it is worth enough to understand “The Importance of executing tasks in the safe OSHE way” and is self explanatory.
The supervisor of the construction site, who told they are experts and have been installing these type of cranes since long time and the word he used was “NO PROBLEM”, But at the end of the day there was a fatality.
Just to highlight and caution ourselves, that over confidence of doing the job because of its repetitive nature and our experience, overseeing safety procedures can result in unwanted results.
May be you are an expert. Accidents occur rarely. That is why they are called ACCIDENTS.
SAFETY ALERT
Near Misses – A simple explanation:
A near miss is an unplanned event that did not result in injury, illness, or damage – but had the potential to do so. Only a fortunate break in the chain of events prevented an injury, fatality or damage. Although human error is commonly an initiating event, a faulty process or system invariably permits or compounds the harm.
For Example
- Potential for Trip and Fall Hazards.
- Blocked/Obstructed access or egress.
- Potential for Fire.
- Substandard work practices which may lead to cause harm (Guards removed from tools, improper/misuse of tools and equipments, bypassing safety devices, substandard work environment etc).
- Defective tools or equipments,
The intension of reporting Near Miss is focus on improvement rather than blaming.
How to Report a Near miss:
The simple way to report a near miss is to send an Email to your organizations Safety Panel , copied to your immediate supervisor giving details of
- Date and time of event.
- Short description of event.
- Immediate Cause of event.
- Your immediate corrective action or suggestion.

ü Pride: Have you read the Japanese story about the janitor who described his work as “Contributing to the progress of his country?” His logic – if the executives did not have clean toilets to use, they couldn’t be very productive, could they? That is the kind of pride you need to have in your work / project.
ü Keep your sense of humor: It helps – especially in situations where no one feels like laughing. (Like the time a short executive stood on a chair so that she was at eye-level with her colleague, and she quipped, “Maybe now we can see eye-to eye?” The laughter that followed this lightened up the tension that everyone in the room had been feeling up to that point.)
